Shambala offers these guided meditations online for free; these are a companion to the book Peaceful Death, Joyful Rebirth; however these meditations can be done without reading the book. I just found them yesterday after reading through the book and then noticing a review online saying that there was a companion audio CD. The meditations involve: seeing the world as pure, visualizing Amitabha, filling oneself with Amitabha's light to purify, praying to Amitabha, visualizing the Pure Land, praying for rebirth in Sukhavati, and sharing the blessings with others.
