Condensed Riwo Sangchod

A long time ago, my Teacher gave me a condensed (du-pa) Riwo Sangchod. At that time, I transliterated the Tibetan into the Roman alphabet. In the several decades since, I seem to have misplaced the original Tibetan (or maybe my Teacher only lent it to me; I don't remember). In any case, I've never had an English translation of this. Most of it I understand, but, nevertheless, I would love to have a "correct" English translation. So my question is, does anyone have an English translation of the following condensed Riwo Sangchod:

SANG-CHOD KUN-ZANG CHO-PAI TRIN-CHEN-PO
LA-MA YI-DAM SANG-GAY JANG-SEM-SANG
PA-WO KHA-DRO CHO-KYONG NOR-LHA-SANG
DRA-LA WER-MA NAY-SHI TER-DAK-SANG
GO-WAI LHA-GON NYAN-PO THAM-CHED-SANG
CHI-NOD NANG-CHU DAK-PA CHEN-POR-SANG

That appears to be Mipham's condensed sang offering.

Your text has a few additional verses. I have been unable to find the pecha online.
Therefore, when one analyzes in detail the final basis for any decision,
Apart from coming back to one’s own mind,
Nothing else whatsoever is perceived

Wow! Thanks. Since posting originally, I had come to decide that this was not a condensed Riwo Sangchod but was some other sang altogether. But I didn't know what sang. Now I do. Great.
