Seven Line Prayer in English, Chagdud Tulku

I once had a copy of Chagdud Tulku's translation of the Seven Line Prayer (that is, the one used in Chagdud Gonpa in the English language Red Tara practice, for instance). I know it by heart in Tibetan, so shamefully, I've lost track of this slip of paper over the years and only now have a need for it. Would someone please post or PM me that version of the seven-line prayer?
