I wonder if someone could suggest a link to an image of Guru Rinpoche in sambhogakaya form? Google doesn't come up with much. Thank you.

If you mean with visible consort, than of course yes, otherwise he is seldom depicted without consort (kathvanga is a symbol of consort/she is hidden in the form of kathvanga).

Here is an example of sambhogakaya Guru Padmakara without visible consort.

Yes, that really is a beautiful thangka. I recognize most of the deities, but what is the form of Guru Rinpoche in the upper left corner, and who is in the upper right corner?

In the upper left corner, that is not a form of Guru Rinpoche, but Dharma King Trisong Deutsen, and the upper right corner is occupied by Drog Kyeuchung Lotsawa - mahasiddha of Dorje Drolo practice and previous birth of Dujom Rinpoche.
